VALIDITY OF SELF-EXCLUSION FORMS

A Self-Exclusion form is strictly only valid if it is completed by correctly filling out one of our company’s standardised forms.

You must not accept or process any form that contains unauthorised alterations. These alterations invalidate the entire form.

1. WHAT MAKES A FORM INVALID?

A form is invalid and must be rejected if the customer has:

Altered Timeframes: Scribbled out, crossed through, or overwritten the printed self-exclusion time periods.

Amended Locations: Hand-written or added any text related to other shop locations, as this printed form only applies to the shop named on the document.

Missing Photo ID: Failed to provide a valid, legible copy of photographic ID for attachment.

2. VALID FORM COMPLETION (ONLY ACCEPTABLE ENTRIES)

A form is only acceptable if the customer has restricted their handwriting to the following necessary elements:

  1. Full Name
  2. Contact Number
  3. Tick Boxes (Selecting markets for exclusion)
  4. Customer Signature and Date
  5. Witness Signature and Date

3. STAFF RESPONSIBILITY

Do not accept or process any form that deviates from the valid completion criteria. If a form is rejected, you must politely explain that the alteration makes the form invalid and immediately provide a new, clean copy for the customer to complete correctly. Compliance with this strict policy is non-negotiable.
