6 Months Self-Exclusion Form

⚠️6 Months is the maximum Self-Exclusion period that can be processed by shop staff.

If a customer requests a self-exclusion period longer than 6 months, you must use the following procedure:

1. Acknowledge and Explain: Politely acknowledge their request for a longer period. Explain that 6 months is the maximum duration that can be actioned immediately in the shop.
2. Refer to Head Office: Inform the customer that requests for longer, non-standard exclusions must be dealt with directly by Head Office.

ℹ️ Note to Staff: Do not attempt to alter a form or create a bespoke agreement. Any Self-Exclusion longer than 6 months requires a dedicated review and handling process initiated by Head Office.
